We are experiencing an issue with MULTIPLE file uploads on web forms. If you need to submit multiple files from a form, please contact the form’s recipient directly (by phone or email).

This is not affecting single file uploads on forms.

The developer of the form engine we use is aware of the problem, and is working on a fix ASAP.
